When should these application forms be used?

If a person who is currently at a resort/safari and would like to get permission to leave.

What are the different options to leave from Resorts or Safaris?

OPTION 1: For resorts/safaris with no cases (suspected or confirmed) at any time

  1. 14 days should have been completed since the exit of the last tourist from the resort/safari, with no reported case (suspected/confirmed) on the resort/safari within this period.

OR

  1. 14 days should have been completed since the entry of the last tourist to the resort/safari, with no reported case (suspected/confirmed) at the resort/safari within this period. If there is any movement during this period (arrivals or departures) 14 days will be counted from the departure date of the last tourist.

OR

  1. The resort staff have completed a 14-day quarantine period at an area designated for this purpose by Resort Management.

OPTION 2: For resorts/safaris under monitoring but with no positive cases at any time

  1. 14-day period should have been completed from the time of the last reported suspected case, with no further cases identified during this period and there are no symptomatic cases on the island at the time of exit.

OPTION 3: For resorts/safaris with reported positive cases at any time

  1. Positive imported case(s) only: 14-day period should have been completed from the time of the last reported positive case. However, if any symptomatic case has been reported during this period, 14 days should be completed from the reported date of the last symptomatic case with adequate active surveillance. Exit questionnaire should be completed with no symptomatic case(s) reported on the island.
  2. Positive case with cluster (a spread to one or more person(s) within the community of the resort/safari): 28-day period should have been completed from the last reported positive case. However, if any suspected case has been reported during this period, 14 days should have passed from the reported date of the last symptomatic case with adequate active surveillance. Exit questionnaire should be completed with COVID-19 testing and no symptomatic or positive case on the island.

 

How do I apply for permission?

OPTION 1 and 2

  • Option 1A or 1B: Resort/Safari management should fill Form No.: HPA/DE/2020/004
  • Option 1C: Resort management should fill Form No.: HPA/DE/2020/001
  • Option 2: Resort/Safari management should fill Form No.: HPA/DE/2020/005

An Excel sheet of list of person(s) leaving from Resort/Safari should be completed and signed by Resort/Safari management. The template for this Excel sheet shall be “Tourist Establishment Information Data Sheet (Option 1 and 2)” as uploaded on our website.

OPTION 3

  • Option 3: Resort/Safari management should fill Form No.: HPA/DE/2020/006.
  • During this period, no one in the Resort/Safari should have any symptoms for COVID-19. This must be monitored by management daily.
  • Upon completion of quarantine period (with no positive cases or suspected cases), all staying at the Resorts/Safaris under Option 3B, should be tested for COVID-19. If the test is negative, all are eligible to leave.
  • To arrange COVID-19 testing, inform HPA through [email protected] at least 3 days prior to completion of the quarantine period.
  • Exit screening must be conducted for all person(s) leaving the establishment. For this purpose, please fill Form No.: HPA/DE/2020/003r.

An Excel sheet of list of person(s) leaving from Resort/Safari should be completed and signed by Resort/Safari management. The template for this Excel sheet shall be “Tourist Establishment Information Data Sheet (Option 3)” as uploaded on our website.

How do I send the forms?

  1. The completed declaration form(s) and Excel sheet should be emailed to [email protected]. The forms and Excel sheet should be sent on or after the declared date.
  2. The subject line for the email should be COVID-19 Release Application [insert tourist establishment name].
  3. All forms must be authorized by Resort/Safari management.

How do I calculate the date of declaration of exit?

  1. Declared date to exit can be based on arrival date if the last tourist (without any COVID-19 like symptoms) stayed for more than 14 days in the resort/safari without any further movement in or out of the resort/safari.

e.g. Arrival date 1st March and departure date is 16th March. In this case the person has completed 14 days within the resort/safari. Therefore, the declared date will be 16th March.

  1. If the last departed tourist has stayed less than 14 days in the resort/safari, the declared date will be based on the last guest departure date. (e.g. Arrival date 1st March and departure date is 10th In this case person has not completed 14 days within the resort/safari. Therefore, the declared date will be 25th March)
  2. If there has been spread within resort/safari, the declared date will be 28 days from the last positive case. During this period if someone develops COVID-19 symptoms then the date will be counted 14 days from the onset of symptoms. However, if the 14 days is within the 28-day period, the declared date will be the date after 28 days.

(1) and (2) are applicable to OPTION 1 and OPTION 2. In OPTION 2, if a suspected case develops a symptom later than that of the last tourist declared time, then 14 days will be counted from the onset date of the last suspected case.

The condition applied is that no person should have COVID-19 symptoms within the 14-day quarantine period (First day will be counted as zero).

Once you are declared, you are entitled to leave from the resort/safari. In addition, movements to-and-from the declared establishment will be allowed unless there is a new tourist check-in. Movement of safaris will be allowed under the same conditions.

Do I need to wait for HPA approval once I submit the forms?

Once you email the completed form(s) and signed scanned copy of Excel sheet mentioned in this guideline, Option 1 and Option 2 do not need any approval from HPA but Option 3 needs approval.
